Vestibular rehabilitation therapy (VRT) is a specialized, exercise-based physical therapy program designed to treat vestibular disorders by promoting adaptation, substitution, and habituation to reduce dizziness, vertigo, improve gaze stability, and enhance balance.
Goals
VRT targets enhancing gaze stability through head-eye coordination exercises, improving postural stability via balance challenges on unstable surfaces, reducing vertigo with habituation to provoking movements, and boosting daily activities. These goals apply to stable vestibular lesions from any cause, age, or symptom duration, with even brief daily sessions yielding benefits.
Key Exercises
- Gaze stabilization: Focus eyes on a target while moving the head side-to-side or up-down, seated then standing.
- Balance training: Stand on foam or narrow base, eyes open/closed, with head turns; progress to tandem stance or single-leg balance.
- Habituation: Repeat motions that trigger dizziness, like quick head tilts, gradually increasing intensity.
- Walking drills: Incorporate head turns or gaze shifts during gait for motion sensitivity.
Therapy starts slowly under professional guidance to minimize fall risk and discomfort, customizing to specific dysfunctions like hypofunction or BPPV.
